Job summary

Bromley Healthcare CIC is seeking an Occupational Therapist (Band 6) to lead the Home Pathway service in the Bromley community. The post focuses on complex assessments, goal-oriented treatment, and developing therapies for elderly patients within their homes and local services.

Qualifications

• Experience with complex assessments and treatment planning.
• Background in rehabilitation and enablement within community settings.
• Ability to supervise junior staff and coordinate team caseloads.
• Experience working with elderly patients and family involvement.

Responsibilities

• Supervision of junior staff
• Individual caseload management and supervision of team caseload management.
• Completing return to works and managing sickness as per the trust’s policy
• Carrying out detailed and timely appraisals in keeping with KPI’s
• Co-lead on the coordination of all discharges from the Home Pathway Team.
• Lead on therapy input in the weekly consultant led MDT.
• Streamline processes to support efficiency within the Home Pathway.
• Service development; a review of current Occupational Therapy services and an innovative approach to developing new aspects of the service.
