Responsibilities
  • Lead care coordination, conduct assessments, support duty rotations, and supervise others--all within a supportive, honest, and integrity-driven team.
  • Coordinate and manage care for service users with complex mental health needs.
  • Serve as Duty Worker on a rotational basis.
  • Conduct initial and follow-up assessments (home visits and clinics).
  • Provide supervision and mentorship to B5 nursing staff.
  • Work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Community Practitioners and Community Healthcare Support Workers where appropriate.
  • Formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their family and carer.
  • Full driving license and access to a vehicle for home visits and community meetings.

Salary

£38,682 to £46,580 a year. Please note for part time hours the salary will be pro rata.
