Please note this role is for 22.5 hours per week.
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co‑ordinate the care of a complex caseload of service users, supervising Community Practitioners and Community Healthcare Support Workers where appropriate.
Working under the supervision of the Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their family and carer.
