What you will do

• Work shifts that span from 8 am to 10 pm, 7 days a week, including weekends and bank holidays.
• Utilize your specialist skills to holistically assess patient needs and develop short‑term care programmes based on evidence‑based practice.
• Focus on service priorities, such as admissions avoidance and timely supported discharge.
• Manage your caseload and support your team with the guidance of the Team Leader.
• Supplement locality‑based planned care teams and collaborate with other health services when acute or intensive care is needed.
• Actively engage in service development activities to enhance care delivery.
