Job summary

A healthcare provider in London is looking for a Band 6 Senior Community Nurse to join their team in Tower Hamlets. You will ensure triaged referrals have a care plan, work collaboratively with locality teams, and manage shift patterns. Essential qualifications include being a Registered General Nurse and having experience in clinical nursing. This is a full-time permanent position that requires strong organizational skills and knowledge of safeguarding processes.

Job Summary

Band 6 - Senior Community Nurse - Tower Hamlets, London

Hours: Full-Time (37.5 hours)

Location: 3rd Floor Beaumont House, Bancroft Road, Mile End Hospital, London, E1 4DG

Contract: Permanent

Working pattern: Full-time

Responsibilities
  • Work as part of a team ensuring all referrals are triaged and have a visit schedule for the appropriate service.
  • Ensure all new admissions to the Community Nursing services have a clinically robust assessment and patient-focused care plan. This is borough-wide cover.
  • Introduce and enable self-management and self-care.
  • Ensure all patients have a safe handover to the professional and team to complete the care plan and measure the outcomes.
  • Partner with locality teams and partner providers to maintain effective patient flow.
  • Work within the financial resources for the patients and the service.
  • Work shifts patterns on a rota: (8am‑4pm, 10am‑6pm and 12pm‑8pm) Monday to Sunday.
Qualifications

Education / Qualification / Training (Essential)

  • Registered General Nurse (RGN)

Desirable

  • DN Certificate / Diploma / Degree
  • Non-Medical Prescriber V300
  • Nurse Formulary prescriber V100
  • Advanced history taking and physical assessment course
Experience

Essential

  • Ability to organize and deliver high standards of evidence-based clinical nursing care.
  • Ability to assess, monitor, plan and evaluate care.
  • Good organisational skills and ability to prioritise work and delegate effectively to the team.
  • Experience of budget management.
  • Ability to liaise effectively with members of the primary care team.

Desirable

  • Experience of managing a team in a health care setting.
  • Experience and knowledge of holistic initial assessments.
Knowledge and Skills

Essential

  • Understanding and knowledge of complex needs and long-term conditions management.
  • Good knowledge of safeguarding processes and ability to raise concerns about vulnerable adults.
  • Knowledge of agencies and organisations within primary care systems that can be utilised to gain optimum care for a patient.
  • Sound knowledge and competence in IT and clinical systems, e.g. EMIS.
  • Knowledge of HR processes for line management of staff.
  • Knowledge of current professional issues facing nursing.
  • Knowledge of community nursing practice and current legislation.
  • Knowledge of information governance and responsibilities.
