Job summary

Birmingham and Solihull Mental Health NHS Foundation Trust is recruiting for a Band 6 Community Mental Health Worker for the 247 Neighbourhood. The post supports adults with severe mental health needs in Small Heath, Bordesley Green and Heartlands wards and involves comprehensive assessment, risk management and collaborative care planning.

You will work autonomously within a multidisciplinary team, engage service users and families, and contribute to recovery-focused care within the community.

Qualifications

• Experience and confidence in working autonomously in an adult mental health setting.

Responsibilities

• Support and engage a caseload of people with mental health problems and social issues. Also to work with their families/carers in collaboration with the wider team.
• Support people experiencing mental health crisis and support their recovery.
• Involved in all aspects of mental health care, assessment, care and support and treatment.
• Support the team in partnering and collaborating with other agencies and organisations in the local community.
• Ensure social inclusion and active citizenship for people with Serious/Severe Mental Illness (SMI).
