Job description
Overview

Senior Community Mental Health Practitioner to join the Community Mental Health Team based at Worsley Court, Selby, to support older adults with mental health needs. The service offers assessment and treatment in a community setting, aiming to promote recovery and living well.

Key Responsibilities
  • Manage your own caseload, providing assessments, care coordination and delivering interventions to people with complex needs ranging from mild to moderate to severe and enduring.
  • Work collaboratively with older adults experiencing functional and/or organic mental health problems, including their carers and families.
  • Provide care coordination, individual assessments and complex intervention planning within the community.
  • Support the implementation of new group services, such as Managing Emotions groups and Carers Support groups, alongside an existing CST.
  • Demonstrate excellent assessment, interpersonal and team‑working skills, working with a range of statutory and third‑sector agencies.
  • Engage in regular appraisal, clinical, management and caseload supervision provided by an experienced multi‑disciplinary leadership team.
Qualifications & Requirements
  • Minimum of 18 months post‑qualifying experience with sign‑off of the appropriate preceptorship or competency framework.
  • Experience working with older adults in a community setting is essential.
  • Ability to travel independently across the locality and access to a suitable vehicle for business purposes.
  • Open to adjustments in accordance with the Equality Act 2010.
  • Employer can offer sponsorship for a Health and Care Worker or Skilled Worker visa where eligible.
