Job summary

Tees Esk and Wear Valleys NHS Foundation Trust is seeking an experienced Senior Community Mental Health Practitioner for the MHSOP Redcar and Cleveland area at Reed Marsh House. This 6-month fixed-term/secondment opportunity involves assessing people with mental ill health and memory decline, with transfer back to primary care and duty rotas.

You will require 18 months post-qualifying experience, sign-off of preceptorship or competency framework, and the ability to travel across the locality

Qualifications

• Current professional registration with an approved professional body.
• Evidence of Continuing Professional Development.
• 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework (at the point of application).
• Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
• Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
• Post-graduate qualification in a relevant area

Responsibilities

• Assess individuals with mental ill health, focusing on cognitive impairment.
• Collaborate with service users, carers and families.
• Manage complex cases in partnership with other agencies.
• Provide clinical supervision to individuals or groups as appropriate.

Senior Community Mental Health Practitioner

An exciting opportunity has arisen for expressions of interest for an experienced Mental Health Practitioner to join our MHSOP Redcar and Cleveland based at Reed Marsh House. This would be for a 6-month fixed term/secondment opportunity. The role will involve assessing people experiencing mental ill health and memory decline from assessment, diagnosis, treatment and transfer back to primary care. The role would involve duty work on a rota basis.

Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the point of application.

Main duties of the job

As a member of the team, you will provide a service to those individuals who present with mental health issues, with an emphasis on assessing for cognitive impairment.You will be willing to work collaboratively with service users of various needs as well as their carers and families.

You will need to have significant experience of working with people with mental ill health in a care setting, delivering clinical interventions specific to care pathways, in particular the Dementia care Pathway. You will possess excellent communication and interpersonal skills. You are required to have the ability and experience to manage complex cases in partnership with other agencies.

You will encourage ongoing engagement from all staff utilising effective leadership skills. You are required to use your own initiative and make decisions independently for this post.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Qualifications

• Current professional registration with an approved professional body.

• For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education.
• 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework (at the point of application).
