Job Overview

This is an exciting opportunity to be part of a dynamic and supportive multidisciplinary team delivering high-quality care to older adults experiencing functional and/or organic mental health needs. You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, and quality of life.

Key Responsibilities
  • Deliver comprehensive assessments, risk formulations, and evidence-based interventions to support older adults with complex mental health needs.
  • Work closely with colleagues across the multidisciplinary team and liaise with GPs, social care, and voluntary sector partners to ensure continuity and quality of care.
  • Maintain accurate and timely clinical records in line with Trust policies, contribute to service development, quality improvement initiatives, and provide professional advice, support, and supervision to colleagues where appropriate.
  • Take responsibility for the management of a complex caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).
  • Participate in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Administer medication in compliance with relevant Trust procedures and NMC Guidance.
  • Assess patient’s individual care needs, develop, implement and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance, including pharmacological and psychologically informed interventions.
  • Complete and contribute to robust risk assessments and risk management plans, promote positive risk taking to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
  • Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
