Job summary

Senior Community Mental Health Nurse

Our Community Older Persons Mental Health Team in Portsmouth is looking for a motivated, enthusiastic Nurse to join our friendly team, who provide skilled assessments and treatment interventions to older adults.

To be a successful applicant you need to be a Registered Nurse, preferably RMN, with experience in the care of older adults with mental health problems. You must be enthusiastic to work with older adults to provide compassionate care to patients and their families, and have knowledge of dementia and functional illness with a good understanding of physical health and frailty.

You will be required to work within the wider MDT team, implementing and evaluating care, managing your caseload, supervising Healthcare Support Workers, junior staff and students within the team. You will play an important role in our team, as we aspire to provide excellent care to our patients and strive to improve and develop our service in line with national and local guidance.

Access to a car for this role and a full driving licence is essential.

Main duties of the job

The Postholder will work within the Older Persons Community Mental health Team providing high quality mental health nursing care to adults over the age of 65 with complex mental health psychiatric conditions / dementia and those under 65 with dementia.

To meet the needs of patients and their families to achieve improved mental health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient client centred, culturally sensitive and evidence-based and which promotes independence.

To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better. This is in line with the requirements set out in the NMC code which require all registered nurses to prioritise people; practice effectively; preserve safety; promote professionalism and trust.
