Detailed job description and main responsibilities
  • Applies evidence-based care to support physical well-being utilising Learning Disability specific and mainstream health targets.
  • Can assess and manage complex clinical issues in relation to service users; can advise and collaborate with carers, professionals, other agencies to around complex clinical interventions and outcomes.
  • Assesses, implements and critically evaluates intervention outcomes to maintain and improve the physical well-being of service users. Involves other agencies/services as appropriate to meet need.
  • Liaises with multi-disciplinary colleagues and other professionals / agencies to address issues that create barriers to social well-being and reports on unmet needs and obstacles to senior colleagues.
  • Develops training for staff, users and carers around health promotion.
  • Facilitate service users and carers in overcoming barriers to accessing primary care and other NHS services.
