Job Summary

An exciting opportunity has arisen for an experienced, forward‑thinking and enthusiastic clinical pharmacist to join our CDDFT pharmacy team. County Durham and Darlington NHS Foundation Trust is one of the largest integrated care providers in England, serving a population of around 600,000 people.

About the Cardiology Team

You will be an integral member of the Cardiology MDT at University Hospital North Durham. The post holder will build the clinical pharmacy service and further develop existing multi‑disciplinary team relationships. Attendance at other sites, including Darlington Memorial Hospital, may be required to deliver key objectives.

Key Responsibilities
  • Deliver a 7‑day pharmacy service, contributing to the on‑call, late and weekend rota.
  • Lead and manage a team of pharmacy staff, ensuring safe, effective and efficient delivery of medicines management services.
  • Develop and support pharmacy services, including research, audit and evidence‑based practice.
  • Represent the directorate at specialist meetings, providing training, specialist advice and expertise.
  • Maintain a portfolio of personal clinical activity commensurate with a senior pharmacist.
  • Employ and promote patient‑centred models of practice that recognise patient and carer needs while maintaining risk minimisation and patient safety.
  • Supervise, support and develop other pharmacy staff, including junior pharmacists and trainees, and provide basic HR support where required.
  • Monitor prescribing and medication use, ensuring it is safe and effective; resolve local prescribing issues and contribute to training of clinical and non‑clinical staff.
  • Work closely with patients, consultant staff and Care Group leads to ensure services meet priorities and needs.
  • Liaise with pharmacy teams, healthcare professionals and external agencies to consider medicine management issues across boundaries.
  • Influence local practice to ensure safe medication use and contribute to strategy, policy development, operational delivery and financial planning.
Essential Qualifications
  • Masters Degree in Pharmacy or equivalent, and completed a suitable postgraduate qualification (e.g., Clinical Diploma).
  • Evidence of CPD and ongoing education.
  • Independent Prescriber or willing to work towards this qualification.
  • Registered as a Practising Pharmacist with the GPhC.
  • Good oral and written communication skills; ability to build relationships with colleagues and influence practice.
  • Experience leading and developing near‑patient clinical pharmacy services.
  • Experience providing directorate pharmacist services, including budgetary reports, drug usage reviews, development of drug protocols and specialist drug information.
  • Experience in audit of clinical practice and multidisciplinary service development.
  • Proficiency in medicines reconciliation, development of clinical guidelines, and understanding of NHS financial pressures and constraints.
Desirable Qualifications
  • Postgraduate Masters qualification in a relevant pharmacy subject and/or postgraduate management qualification.
  • Working towards an advanced practice development portfolio as defined by the RPS.
  • Teaching experience in clinical and classroom settings; experience in providing basic HR support.
  • Evidence of research, audit and scholarly publications.
  • Membership of RPS or another relevant clinical pharmacy organisation.
  • Experience managing a team and working across the NHS.
  • Good presentation skills.
