Job summary

Qualifications

• Must be first level registered Mental Health Nurse.
• Post registration qualification in Child and Adolescent Mental Health.
• Evidence of working in in-patient Adolescent Mental Health.

Responsibilities

• Act as case manager for a small group of young people.
• Work closely with the multidisciplinary team to plan and deliver care.
• Support young people's discharge back to the community.
