Senior AHP Support Worker (Surgery) (Rehabilitation) role within the Royal Devon NHS Foundation Trust in Exeter, a permanent, full-time position providing therapeutic interventions, treatments and exercise programmes to patients and liaising with patients and family/carers in discharge planning and falls prevention.

Post holders will support the therapy team with early routine assessments and rehabilitation, and work with multidisciplinary teams across acute and community settings.

Responsabilidades

• Provide a range of therapeutic interventions, treatments and exercise programmes to patients.
• Liaise with patients and family/carers in discharge planning and prevention of falls.
• Identify physical limitations of patients' disabilities to support therapy team in early assessments and rehabilitation.
• Develop strong links with multidisciplinary teams in acute and community settings.
• Manage caseload of surgical patients being assessed for discharge home or to their normal residence.
• Work across all areas and be available for shifts across 7 days including weekends and bank holidays.
