SEHSCT Home Treatment Team Senior Practitioner

The post holder will have responsibility for providing specialist nursing skill, knowledge and expertise in the care and management to patients referred to the Home Treatment Team. The post holder will participate in the co-ordination of gate keeping of acute care referrals from a wide range of services. The post holder will facilitate the provision of expert mental health nursing intervention and support to service users and their families and will undertake and participate in audit, research and development of the Home Treatment Team. The post holder will be an advocate to service users and their families promoting person centred care and act as a resource for healthcare professionals involved with these service users.

Responsibilities
  • The post holder will provide leadership to other professionals through provision of clinical practice, education, research and leadership to achieve the best quality of life and outcomes for service users in the care of the Home Treatment Team.
  • To take the lead in the implementation, evaluation of care and the setting of standards of care for patients in collaboration with the multi‑disciplinary team.
  • The post holder is expected to provide clinical leadership to staff within the Home Treatment Team.
  • The post holder will provide increased senior clinical and decision making advice to the Home Treatment Team.
Skills and Qualifications
  • Minimum of 5 years post basic qualification.
  • Minimum two years’ experience working at Band 6 or above in an Adult Mental Health Setting.
