Job summary

NELFT NHS Foundation Trust is recruiting a Band 6 Health Visitor in Thurrock to deliver the Healthy Child Programme and work within a multidisciplinary team to improve outcomes for children and families in the borough. Newly qualified practitioners can join the Preceptorship Programme, with extensive in-house training and monthly supervision, mentorship, and annual appraisals to support development.

Thurrock has an exciting opportunity for Band 6 Health Visitors. If you are an experienced Health Visitor, or soon to be qualified Health Visitor, we would love to hear from you.

We are looking to recruit motivated practitioners with initiative and drive who are keen to contribute to the further development of our service. We work in close collaboration with Family Hub’s as well as other agencies to improve outcomes and reduce health inequalities for children, young people and their families in our borough.

For newly qualified practitioners, we have a Preceptorship Programme in place to aid your development. We also offer a wide range of ‘in-house’ and other training opportunities for continuous professional development through our learning and development department.

We provide monthly clinical supervision, mentorship and yearly appraisals to support you in your role and development. We are an inclusive organisation and are keen to celebrate the success and hard work provided by practitioners in improving the health outcomes for children we work with.

Main duties of the job

To provide key contacts as per the Healthy Child programme, identifying and assessing health needs. Working within the wider multidisciplinary team to provide appropriate support for families.

To identify and assess children 0-5 with complex health needs, offering an enhanced integrated service within the multi agency team.

To identify possible safeguarding concerns and escape for support as required, working with other agencies across the borough.

You will need to have the ability to work autonomously and as part of an integrated skill mix team. You need to have excellent communication skills and experience of working in partnership across organisational boundaries. You need to be willing to facilitate and support change in service delivery.

Detailed job description and main responsibilities

To lead the delivery of the Healthy Child Programme, and ensure the 4, 5, 6 Service model is implemented in practice.

Ensure services are provided within the HCP model- Your Community, Universal, Targeted and Specialist Services This will incorporate building community capacity, and embedding this into caseload, team and locality management.

Responsible for the management of caseload allocation as per locality/team, and classify cases in line national stratify of Universal, Universal plus and Universal partnership plus.

Work in accordance to the Locality commissioned service specification, to ensure the 5 mandated reviews are completed within required timescales, services provided are evidence based practice and the 6 High impact areas are embedded within practice.

To improve the health and wellbeing of children and young people especially those from disadvantaged groups.

To Identify, prioritise, develop and implement programmes of care to meet individual and local needs, taking into account cultural diversity. This will be achieved through evidence based assessments, effective care planning and implementation of a range of support packages for individual families, targeting the most vulnerable families and groups utilising best available evidence .

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Fringe

(Basildon, Thurrock, Brentwood, Harlow and Dartford only)

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,303 to a maximum of £2,198 p.a. pro rata for part time)
