Job summary

Spinney Brook Medical Centre in East Northants invites a GP to join our stable, collaborative team. We offer a modern access model and a supportive multidisciplinary environment, welcoming both newly qualified and experienced GPs.

You will provide patient-centred care across acute and long-term health needs, participate in New Models of Care, and work with GPs, nurses, pharmacists and others to deliver high-quality services.

Qualifications

• Fully qualified GP with GMC registration and a current licence to practise in England.

Responsibilities

• Ideally working between 4 and 7 sessions per week.
• Provide high-quality, patient-centred general medical services as part of a multidisciplinary primary care team.
• Undertake clinical assessment, diagnosis and management of patients with a wide range of acute, urgent, chronic and complex health needs, ensuring safe and effective clinical decision-making.
• Support the delivery of same-day and urgent care, including seeing patients with acute problems and undertaking clinical triage of patient requests through Anima Total Triage during allocated sessions.
• Support continuity of care and the management of patients with long-term conditions, frailty and complex health needs.
• Contribute to the practice's New Models of Care work, supporting proactive and coordinated care for patients who are frail, have multiple or complex long-term conditions, are approaching the end of life or make frequent use of secondary care services.
• Work collaboratively with GPs, nursing staff, pharmacists, paramedics, healthcare assistants, administrative colleagues and the wider multidisciplinary team to deliver safe and effective services.
• Participate in multidisciplinary discussions and care planning, including working with colleagues across the East Northants Neighbourhood and, where appropriate, specialists from hospital and community services.
• Maintain accurate and contemporaneous clinical records and make appropriate referrals, investigations and follow-up arrangements.
• Contribute to quality improvement and the ongoing development of services within the practice, where appropriate.

We are looking for an enthusiastic and committed GP to join our supportive clinical team. Applications are welcomed from both newly qualified and experienced GPs. Newly qualified GPs will be supported as they establish themselves within a friendly and approachable team.
The role offers a varied mix of general practice, supporting the health needs of our whole practice population. You will provide care for patients with acute, on-the-day problems as well as those with ongoing, complex and long-term health needs.
There will also be the opportunity to become involved in our New Models of Care work, focusing on patients with long-term conditions, frailty, complex needs, end-of-life care and those who make frequent use of secondary care services.

Main duties of the job

The post holder will provide safe, effective and patient-centred general medical services as part of our multidisciplinary team.
The role will include managing a varied clinical workload and providing care for patients with acute, urgent and ongoing health needs. You will undertake face-to-face, telephone and other appropriate consultations, assessing, diagnosing and managing patients across the full range of general practice.
An important part of the role will be supporting same-day care. This will include seeing patients with acute problems and acting as the clinical triaging GP for Anima Total Triage during allocated sessions. You will review patient requests, assess clinical need and urgency, and ensure patients are directed to the most appropriate clinician or service.
Alongside acute care, there will be opportunities to support patients with long-term conditions and complex health needs through our New Models of Care work. This focuses on patients who are frail, living with complex or multiple conditions, approaching the end of life or making frequent use of secondary care.
You will work closely with GPs, nurses, pharmacists, paramedics, healthcare assistants and other professionals to provide coordinated, proactive and patient-centred care.

This will include multidisciplinary team meetings involving colleagues from across the Neighbourhood, alongside specialists from hospital and community services. These MDTs will support coordinated care planning for patients with complex needs and provide an opportunity to work collaboratively across organisational boundaries.
Within the practice, Anima Total Triage supports effective management of patient demand and helps ensure patients are directed to the most appropriate clinician or service.
