You will work alongside our GP team, experienced nursing and paramedic teams, and our wider Primary Care Network (PCN) team, including physiotherapists, mental health practitioners, social prescribers and clinical pharmacists.

We work as a multidisciplinary team, with GPs, experienced nursing and paramedic teams, supported by our Ascent Primary Care Network (PCN). Our wider MDT includes physiotherapists, mental health practitioners, social prescribers and clinical pharmacists, enabling patients to access a broad range of expertise within primary care.
We are also a training practice, regularly supporting GP Registrars, F2 doctors on rotation and trainee nurses. To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the GMS contract.

A full induction programme is provided at Walnut Tree Health Centre. When attending any practice within the network, the post holder must also complete that practice's induction programme.

All staff must complete mandatory training as directed by the training co-ordinator. The post holder is expected to assess their own learning needs and undertake appropriate learning.
The post holder will undertake mentorship for team members and share learning and information to promote good practice and inform colleagues of current and future developments, including courses and conferences.
The post holder will provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.

All staff should delegate clearly and appropriately, following safe practice and assessment of competence. Plans and outcomes by which to measure success should be agreed.

Data should be reviewed and processed using accurate SNOMED/read codes to ensure easy and accurate information retrieval for monitoring and audit.
Staff will receive detailed information during induction regarding policies and procedures. The post holder must adhere to organisational policies and regional directives and ensure protocols are followed.
Security is the responsibility of all personnel. The post holder must remain vigilant and report suspicious activity immediately to their line manager.
Door lock codes must never be shared. Restricted areas must remain secured. Password controls must be maintained and passwords must not be shared.
All staff are required to dress appropriately for their role.
