Job summary

Allscreens Nationwide Ltd. is seeking a healthcare professional to join our patient-centred team. You will conduct consultations, assess and manage health conditions, and work with GPs and hospital teams to ensure holistic patient care.

You will maintain accurate clinical records, participate in audits and training, and support service delivery and patient experience while upholding safeguarding standards and promoting preventive care.

Responsibilities

  • Conduct consultations with patients to assess, diagnose, and treat various health conditions
  • Managing chronic illnesses and providing preventive care
  • Coordinating with other healthcare professionals to ensure holistic patient care
  • Maintaining accurate and detailed patient records
  • Participating in practice meetings and contributing to the continuous improvement of services
  • Supporting and mentoring junior staff and medical trainees
  • Provide same-day assessments, diagnosis and management for patients presenting with urgent but non-life-threatening conditions
  • Deliver high-quality, evidence-based care in line with clinical guidelines and local pathways
  • Undertake clinical examinations, investigations, and procedures within your competence
  • Prescribe safely and appropriately in accordance with national and local protocols
  • Identify and escalate patients requiring emergency intervention or secondary care
  • Ensure continuity of care by communicating effectively with GP practices, community services, and hospital teams
  • Maintain accurate, timely clinical records using the centres electronic patient record system
  • Participate in clinical audits, quality improvement activities, and service reviews
  • Engage in regular appraisal, revalidation, and mandatory training
  • Ensure adherence to safeguarding policies for adults and children
  • Contribute to Practice discussions and collaborate with ANPs, Nurses, HCAs, and Reception/Admin staff
  • Support the centre in meeting service standards, KPIs, and CQC requirements
  • Provide a friendly, respectful, and efficient service to all patients
  • Promote health education, self-care, and preventative approaches
  • Help maintain a safe, clean clinical environment, escalating issues where needed
