Job description

The purpose of this role is to work as a Roving Health Care Assistant, providing qualityclinical and administrative support in an efficient manner to patients in aresidential, care home or local setting, under the direction of the MedicalDirector.

You will be expected to travel within Tower Hamlets, Newham andWaltham Forest and Havering

Main duties of the job
  • Qualified Health Care Assistant to travel across different boroughs
  • To provide a professional service to our multilingual diverse practice population using available resources to aid effective communication
  • Work closely with GP/PCN Care Coordinators / GP Assistants to identify patients who have been lost to follow up, and are not being served by the current diabetes service or SMI register. This includes patients who are of working age, LD, vulnerable, housebound, complex care requiring MDT approach, language barriers, concurrent mental health problems etc
  • Write back/discharge to GP, unless updating EMIS directly
  • Liaise with participating practices to book patients for reviews
  • Complete appropriate onward referrals e.g., to foot health, structured education, weight management programmes etc.
  • Provide lifestyle advice including smoking cessation, weight management, diet and exercise
  • Support patients in ongoing referrals/signposting to other services e.g., dietitian service, gym, walking groups etc
  • Consider the mental health and wellbeing of patients and refer on to social prescribers/ wellbeing coaches/ Talking Therapies as appropriate
  • Work with social prescribers, care navigators, Diabetes UK and other health and social care professionals
  • Undertake other clinical activities

Job responsibilities

Operational

  • Work closely with GP/PCN Care Coordinators / GP Assistants to identify patients who have been lost to follow up, and are not being served by the current diabetes service. This includes patients who are of working age, LD, vulnerable, housebound, complex care requiring MDT approach, language barriers, concurrent mental health problems etc.
  • Increase 8 Care processes data collection
  • Present patients at a diabetes MDT if required
  • Write back/discharge to GP, unless updating EMIS directly
  • Liaise with participating practices to book patients for reviews
  • Complete appropriate onward referrals e.g., to foot health, structured education, weight management programmes etc.
  • Provide lifestyle advice including smoking cessation, weight management, diet and exercise
  • Support patients in ongoing referrals/signposting to other services e.g., dietitian service, gym, walking groups etc
  • Consider the mental health and wellbeing of patients and refer on to social prescribers/ wellbeing coaches/ Talking Therapies as appropriate
  • Provide information on national and local diabetes programmes and structured education
  • Supporting recent discharges from hospital/liaison with DSN
  • Work with social prescribers, care navigators, Diabetes UK and other health and social care professionals
  • Shared decision making with patients, involving personalised care roles such as health and wellbeing coaches or social prescribers as appropriate
  • Undertake other clinical activities identified in accordance with written guidelines
  • Decontamination and sterilisation procedures

Administrative/Clerical Duties

  • Maintain infection control standards
  • Updating medical records
  • Updating practice team of any concerns raised by patients that cannot be resolved during the HCA consultation

Communication and Record Keeping

  • Maintain accurate written and computerised records in line with Practice/PCN/NHS NEL information sharing policies, procedures and guidelines
  • Collect and prepare statistical information as required using existing recording systems in practices/PCNs
  • Attend and participate as appropriate in meetings and communicate effectively outside of meetings

Personal/Professional Development

  • The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include:
  • Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
  • Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work
  • Attend in-service training and other training as required
  • Identify own training needs to enhance quality care
  • Assist with the training and development of other personnel as required and as requested by the line manager.
Person Specification
Qualifications
  • Qualified HCA
  • Specific training in diabetes, foot checks and associated care
  • A good understanding of and be able to explain diabetes as a disease, how medicines to treat the condition work and their side effects
Experience
  • Maintain confidentiality at all times
  • Act as a chaperone (after appropriate training) as and when necessary, in line with the practice chaperone policy documenting every occasion in the patients records using the practice chaperone template.
  • Maintain health and safety at all times

  • This is a roving role where you will be expected to travel to different location within Waltham Forest - Tower Hamlets - Havering and Newham.
