A healthcare organization in the United Kingdom is seeking a Band 5 postholder to manage a designated caseload of patients across community clinics and homes. The role involves assessing and managing patients with diverse conditions, ensuring high-quality care and safe practices. Candidates should demonstrate compassion and dedication in providing care, while working effectively within multidisciplinary teams. This position is part of a rotational scheme, offering diverse clinical experiences across various locations.

As part of the Band 5 rotational scheme, the postholder will work across community clinics in the hospital, outpatient locations and patients' homes, gaining broad experience and developing clinical skills. Each six‑month rotation builds on previous experience, enabling the postholder to strengthen and expand their clinical competencies with guidance and support from senior staff.
All staff are supported by our award‑winning Preceptorship team to complete their preceptorship within their first year. There are many opportunities for training and development even as a new member of staff and we welcome you to join our strong communities of practice.
Rotations include Musculoskeletal outpatients in Bexley Care, Musculoskeletal physiotherapy in Greenwich Clinics, COPD/pulmonary rehabilitation, Community rehabilitation, community neurology and our Meadowview intermediate care unit.
The main duties of the job are to assess and manage patients referred to the team with a wide range of conditions.
