Oxford University Hospitals NHS Foundation Trust is offering a 12-month specialist pharmacist rotation across multiple clinical areas in Oxford. You will develop in four pillars of practice, mentoring others and engaging in service development or research within a supportive environment.

The role emphasizes medicines management, two-way information exchange with clinical teams, and opportunities to pursue Non-medical Prescribing upon registration with the GPhC.

Formación

• Master of Pharmacy (MPharm) completed; registered pharmacist or eligible for GPhC registration.
• Postgraduate Certificate in Pharmacy Practice and diploma/credentialling in CPPE/RPS or equivalent.
• UK professional registration required.

Responsabilidades

• Deliver compassionate patient-centred clinical pharmacy service.

• Ensure timely medicines reconciliation and transfer of care to GPs/community pharmacy.
• Support antimicrobial stewardship, medicines safety and efficacy initiatives.
• Complete training in clinical trials and support supply of investigational products as needed.
• Pursue Non-medical Prescribing where scope exists within rotations.
• Liaise with OUH dispensaries and external providers to ensure safe medication supply.

Building on your foundation knowledge, experience and skills, you will be working in a dynamic and rewarding setting to deliver compassionate excellence. As a specialist pharmacist you will grow in all four pillars of practice in order to prepare for an advanced practice role. In this post, you will have 12-month rotations across a range of clinical areas. The one year rotation allows you ample opportunity to be become integrated in each new clinical area and expand your expertise and skillset, to mentor others, be involved in service development or research and to put your management and leadership skills into action in a supportive setting. The post holders should ensure timely, effective, two-way exchange of information between the pharmacy team and the clinical area as well as maintaining high standards of medicines management. Each rotation has a specific set of objectives which will be shared prior to annual rotation selection and agreed at the start of the rotation.

Direct Patient Care

1. Deliver compassionate excellence via a patient focused clinical pharmacy service, in accordance with national medicines optimisation principles and local clinical pharmacy procedures.
2. Analyse prescriptions, alongside technical resources, patient records and information from patient consults to form an opinion on the most appropriate course of action.
3. Accurately and sensitively communicate medicines related information to a variety of healthcare professionals and patients including those with language difficulties, physical or mental disabilities, in a way that facilitates shared decision making.
4. Optimise transfer of patient care, though timely completion of medicines reconciliation and communication with GPs and community pharmacy teams.
5. Participate in and promote antimicrobial stewardship, medicines safety, and medicines effectiveness initiatives.
6. Where relevant to a rotation complete training in clinical trials and support the supply of investigational medicinal products.
7. Where a scope of practice has been identified within a rotation, train to become a NMP and when registered with the GPhC and the OUH to prescribe within the Trust's policy for Non-medical Prescribing.
8. Support OUH dispensaries, and liaise with external agencies (Such As aseptic production services or homecare providers) to ensure safe, timely and appropriate supply of medications to patients.

Qualifications

• Masters Degre (4 Year MPharm or equivalent) in Pharmacy
• Registered as pharmacist with the GPhC
• Post Graduate Certificate in Pharmacy Practice and due to complete the diploma, or equivalent external credentialling via CPPE/ RPS

Desirable qualifications

• Post Graduate Diploma/MSc in Pharmacy Practice or experience/ qualification/ credentialling deemed equivalent.
• Independent Prescriber
• Completion of formal Leadership program, Direct patient care in more than one one setting or clinical area.
• Working with other health care professionals
• Communicating complex information via multiple methods to a variety of recipients.
• Quality improvement, service evaluation or audit
• Mentoring and training others in a healthcare setting
• Taking the lead in a team or project
• Hospital experience in more than one setting or clinical area
• Experience in Pharmaceutical care of patients in critical care or enhanced care setting
• Appreciation of funding streams for medicines budgets.
• Demonstrating measurable outcomes from quality or service improvement
• Preparation of protocols or guidelines
