We arerecruiting an enthusiastic Asthma/COPD Nurse to work across Lakeside Surgery,Corby and Brigstock.

Do you have experience in managing a long-term condition such as Asthma/COPD within a Primary Care environment? Or are you an experienced Practice Nurse who has expanded their skills and has experience in managing patients with Asthma/COPD?

As an Asthma/COPD nurse,you will provide and maintain a high standard of chronic disease management,which includes diagnosis, monitoring and development of individual managementplans and agreeing these as appropriate with the patient and other healthprofessionals

Main duties of the job

The post holder will work collaboratively within the multi-disciplinary general practice team, to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will provide specialist assessments and reviews of health care needs, the optimisation of care where co-morbidity exists, maximising wellbeing, self-empowerment, and quality of life.

The post holder will be expected to:

  • Responsible for managing and running Asthma/COPD clinics
  • Be proactive in QoF and enhanced services management by completing annual reviews using Ardens templates and patient digital platforms
  • Review patients who present to the service on the day needing an acute review
  • Process pathology, workflow, tasks and assisting with audits as required

Job responsibilities

As an Asthma/COPD Nurse you will provide high-quality, evidence-basedcare to patients with asthma and other respiratory conditions. The roleinvolves diagnosis, monitoring & development of individual managementplans, agreeing these as appropriate with the patient & other healthprofessionals. You will be responsible for supporting patients in managingtheir condition, promoting self-care, and ensuring adherence to local andnational asthma management guidelines (e.g. BTS/SIGN, NICE). Working within theNMC Code of Conduct and professional boundaries, the post holder will deliverprimary health care interventions and care. The post holder will supportservice development, the safe delivery of policy and procedures, and demonstrateeffective professional nurse standards, advocacy, and performance.

  • Registered Nurse
  • Evidence of continued professional development
  • Mentorship course or equivalent
  • Accredited Diploma Level training or post-graduate certificate in Asthma/COPD/Respiratory management
  • Primary Care Nursing qualification
  • Clinical Supervision Training & experience
Experience
  • Recent experience as Registered Nurse/Nurse Practitioner in a Primary Care setting
  • Current experience of working in an independent and semi- autonomous role, diagnosing and treating patients
  • Involvement in implementing and using protocols and clinical guidelines
  • Experience of managing/dealing with the public/patients
  • Proactive in improving and influencing the patient care pathway/ journey.
  • Demonstrates the ability to manage clinical risk and understands health care governance systems
  • Experience of using clinical systems & Microsoft Office software
  • Participation in quality initiatives such as clinical benchmarking
  • Experience of audit and/or research
  • Experience of SystmOne
