A healthcare provider in England is seeking an enthusiastic Asthma/COPD Nurse. The role involves managing Asthma/COPD clinics, delivering high-quality care, and supporting patient self-management. We arerecruiting an enthusiastic Asthma/COPD Nurse to work across Lakeside Surgery,Corby and Brigstock.
Do you have experience in managing a long-term condition such as Asthma/COPD within a Primary Care environment? Or are you an experienced Practice Nurse who has expanded their skills and has experience in managing patients with Asthma/COPD?
As an Asthma/COPD nurse,you will provide and maintain a high standard of chronic disease management,which includes diagnosis, monitoring and development of individual managementplans and agreeing these as appropriate with the patient and other healthprofessionals
The post holder will work collaboratively within the multi-disciplinary general practice team, to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will provide specialist assessments and reviews of health care needs, the optimisation of care where co-morbidity exists, maximising wellbeing, self-empowerment, and quality of life.

Lakeside Surgery 46500 patients registered and is rated as GOOD with CQC. We have a strong team ethos and believe that including those delivering the service in decisions are key to the best patient outcomes so management actively engage with all parts of the surgery to achieve this.
We continue to invest in the training and development of our nursing team and recognise the importance of personal as well as organisational development.
As an Asthma/COPD Nurse you will provide high-quality, evidence-basedcare to patients with asthma and other respiratory conditions. The roleinvolves diagnosis, monitoring & development of individual managementplans, agreeing these as appropriate with the patient & other healthprofessionals. You will be responsible for supporting patients in managingtheir condition, promoting self-care, and ensuring adherence to local andnational asthma management guidelines (e.g. BTS/SIGN, NICE). Working within theNMC Code of Conduct and professional boundaries, the post holder will deliverprimary health care interventions and care. The post holder will supportservice development, the safe delivery of policy and procedures, and demonstrateeffective professional nurse standards, advocacy, and performance.
