A community health service provider is seeking a Respiratory Specialist Nurse to enhance care for patients with respiratory issues. An exciting opportunity has arisen for a well-motivated, enthusiastic Respiratory nurse to work for South West Yorkshire Partnership NHS Trust within the BREATHE service. This community-based team provides specialist assessment and review of patients with a confirmed respiratory diagnosis.
The service is responsible for delivering specialist care to patients in their own home, as well as in our community clinics and venues around Barnsley. The successful candidate will assess and treat patients with a wide variety of respiratory conditions. The team is committed to personal development, training & wellbeing. While this role meets the eligibility requirements for Skilled Worker sponsorship, due to the short-term nature of the contract the Trust will not be offering sponsorship for this post.

Work as an expert practitioner, demonstrating evidence-based knowledge, analytical skills, and specialist assessment to triage, assess and manage a range of complex presentations.
Work autonomously assessing and managing your own clinical caseload supporting patients with a range of respiratory conditions in a community setting, including those requiring End of Life care and home oxygen assessment and therapy.
Fully integrate within the aligned neighbourhood and play an integral role in the Respiratory pathway. Participate in MDTs, collaborate with primary care colleagues, and work with respiratory and medical teams within secondary care and the virtual ward to facilitate early discharge and care at home, providing seamless care.
Work as part of a team and as a lone worker, allocated to a specific neighbourhood, with travel and work across all neighbourhoods required.
