An exciting opportunity has arisen for a well-motivated, enthusiastic Respiratory nurse to work for South West Yorkshire Partnership NHS Trust within the BREATHE service. This community-based team provides specialist assessment and review of patients with a confirmed respiratory diagnosis.

The service is responsible for delivering specialist care to patients in their own home, as well as in our community clinics and venues around Barnsley. The successful candidate will assess and treat patients with a wide variety of respiratory conditions. The team is committed to personal development, training & wellbeing. While this role meets the eligibility requirements for Skilled Worker sponsorship, due to the short-term nature of the contract the Trust will not be offering sponsorship for this post.

Main Duties

Work as an expert practitioner, demonstrating evidence-based knowledge, analytical skills, and specialist assessment to triage, assess and manage a range of complex presentations.

Work autonomously assessing and managing your own clinical caseload supporting patients with a range of respiratory conditions in a community setting, including those requiring End of Life care and home oxygen assessment and therapy.

Fully integrate within the aligned neighbourhood and play an integral role in the Respiratory pathway. Participate in MDTs, collaborate with primary care colleagues, and work with respiratory and medical teams within secondary care and the virtual ward to facilitate early discharge and care at home, providing seamless care.

Work as part of a team and as a lone worker, allocated to a specific neighbourhood, with travel and work across all neighbourhoods required.

Job Summary
  • The post-holder will support the Team Leader in the delivery of the Breathe Service in community.
  • Manage a defined clinical caseload, providing specialist support, education and complex case management to people with respiratory conditions and their families and carers.
  • Work as part of a wider Neighbourhood Multi‑disciplinary team of nursing, therapy, mental health, social care and primary care staff.
  • Liaise with primary and secondary care colleagues to provide education and support, thereby developing and maintaining high quality evidence‑based respiratory care across Barnsley.
Key Result Areas
  • Work autonomously assessing and managing own clinical caseload for all clients/patients within a designated neighbourhood (both clinic and home visits). Support patients with a range of respiratory conditions or on a respiratory rehabilitation pathway, including end‑of‑life support and long‑term oxygen or nebuliser assessment and therapy.
  • Use and apply specialised skills underpinned by advanced theoretical knowledge of professional practice to maintain professional competence and fitness to practice as a Respiratory Nurse Specialist.
  • Demonstrate evidence‑based knowledge, investigative and analytical skills and specialised assessment techniques to triage, assess and manage a range of complex presentations.
  • Formulate and implement treatment plans based on current evidence and expert clinical reasoning, integrating self‑management principles to enable individuals to function independently and reduce inappropriate hospital admissions.
  • Enable people to receive an early and accurate diagnosis and provide relevant information and support to manage their respiratory disease.
  • Participate in multi‑disciplinary team meetings, contributing to decisions regarding patient care.
  • Work alongside Primary Care GPs and Practice Nurses, providing targeted support to practices with high respiratory disease rates.
  • Collaborate with respiratory and medical teams within secondary care to facilitate early supported discharge of patients following an emergency admission.
  • Manage referrals to the service from medical, nursing staff and other sources, ensuring patients are seen within required timescales based on clinical need.
  • Initiate, plan and implement induction, training and education of junior team members, the broader community nursing teams and students.
  • Provide appropriate supervision and caseload support for junior members.
  • Understand National Standards such as NICE guidelines, National Service Frameworks and British Thoracic Society guidelines where applicable.
  • Lead on the development and delivery of specialised education programmes for professionals.
  • Contribute to policy, procedure and standard development and review, using an evidence‑based approach.
  • Contribute to the clinical governance framework to maintain high standards of care.
  • Undertake nurse prescribing in response to assessed need, in accordance with trust policy.
  • Maintain complete, accurate and up‑to‑date records in accordance with NMC guidelines and trust policy, recording all client contacts and activities promptly in the IT system.
  • Adhere to the NMC scope of professional practice and trust policies, ensuring professional competency and development by updating skills and knowledge and attending all mandatory training.

Communicate effectively with all members of the Breathe Service to provide a cohesive and effective service.

Qualifications (Essential)
  • First level Nurse.
  • Graduate level education and/or working towards Masters level.
  • Diploma/Degree in Respiratory Disease or working towards.
Qualifications (Desirable)
  • Professional teaching qualification.

  • Recent up-to-date knowledge of COPD, oxygen and respiratory care, including management of acute exacerbations.
