Job summary

Wirral University Teaching Hospital NHS Foundation Trust seeks a Clinical Nurse Specialist to join the Wirral Integrated Respiratory Service for a 12 months fixed term contract or secondment. The service delivers early supported discharge and admission avoidance for respiratory patients, with a team including Specialist Nurses, Assistant Practitioners, Physiotherapists and more.

The role involves holistic assessment, care planning, and working with partners to develop the future of respiratory

Qualifications

• Perform holistic assessment of patient nursing needs
• Plan, implement and evaluate care delivery according to changing health care needs
• Identify opportunities to promote and provide health education for patients and carers
• Advise on self-management strategies including medicines management, exercise, smoking cessation, dietary issues

Responsibilities

• Assess, review and follow up respiratory patients in clinics and homes when virtual ward capacity allows
• Manage patients discharged from hospital with acute exacerbation of their chronic respiratory condition in their own homes
• Work collaboratively with other professionals and agencies to ensure patient needs are met
• Collect, collate, evaluate and report information, maintaining accurate patient records
