Job summary

Homerton Healthcare NHS Foundation Trust is seeking a Band 6 Physio or Nurse to join the ACERS Respiratory Team, providing care in the community and hospital settings across City and Hackney. The role supports people with chronic cardiorespiratory conditions and improves outcomes within a seven-day service.

Applicants must have UK registration (NMC/HCPC) and appropriate professional qualification in nursing or physiotherapy, with NHS experience preferred.

Qualifications

• Effective verbal and non verbal communication skills.
• IT literate and able to use electronic health records.
• Experience working with respiratory patients preferable but not essential.

Responsibilities

• Support patients with COPD, asthma, bronchiectasis and ILD in the community and hospital discharge scenarios.
• Provide rapid response services to prevent unnecessary hospital admissions and support early discharge.
