Ideal candidates have hospital pharmacy experience (2+ years) and a PharmD with active license. You may influence outcomes by flagging inappropriate drug choices and communicating updates to prescribers.

Qualifications

• 2+ years of professional pharmacy experience, inpatient/hospital setting preferred.
• Frequent Lexidrug or Lexicomp use weekly or more in your current or recent role.
• Reflexive use of interactions, Trissel's, and monograph sections.
• PharmD and active pharmacy license, or equivalent verification-heavy clinical experience.
• Strong working knowledge in at least 2–3 specialized areas above.

Responsabilités

• Confirm interaction, dosing, or contraindication findings against the patient's full medication list and chart.
• Catch missed interactions, stale patient data, and monograph guidance superseded by newer Special Alerts.
• Judge completeness of the clinical picture, ensuring all active meds, labs, and comorbidities are considered.
• Distinguish 'permitted' from 'appropriate' and flag poor drug choices for specific patients.
• Verify that clinical conclusions are carried through with order updates, prescriber notifications, and documentation.
