Ortus Health is seeking a dedicated and experienced Remote Clinical Pharmacist to join our innovative healthcare team. In this fully remote role, you will provide high-quality care and support to patients within a collaborative primary care model, enhancing medication management, improving patient outcomes, and ensuring safe prescribing practices.

Responsibilities
  • Conduct comprehensive medication reviews for patients, focusing on safety, efficacy, and adherence.
  • Provide clinical advice on medication therapy management to patients and healthcare providers.
  • Collaborate with multidisciplinary teams to ensure optimal patient care and therapeutic outcomes.
  • Monitor and evaluate patient responses to medications and recommend adjustments to therapy as needed.
  • Stay updated with current pharmaceutical and clinical guidelines and contribute to the development of practice protocols.
  • Participate in continuous professional development and training opportunities.
Qualifications
  • Registered pharmacist with current GPhC registration.
