Job summary

Medacs Healthcare is recruiting a CHC Nurse Assessor (Band 6) for a fully remote locum role in the UK. The post involves 37.5 hours per week, Monday–Friday, assessing care needs and coordinating packages while liaising with NHS services and local authorities.

The ideal candidate will have CHC assessment experience, current NMC registration (RGN/RMN/RNLD), and the ability to manage a caseload with strong clinical decision-making and communication skills.

Qualifications

• Experience conducting CHC assessments and reviews.
• Current NMC registration as RGN, RMN or RNLD is essential.
• Ability to manage a caseload and coordinate care packages.

Responsibilities

• Complete CHC assessments and reviews.
• Manage a caseload of individuals with complex health needs.
• Review and coordinate packages of care.
• Support NHS-funded Nursing Care and Fast Track pathways.
• Maintain accurate clinical records and documentation.
