Responsibilities
  • Serve as the primary remote point of contact for patients, addressing inquiries and providing information regarding services and care options
  • Coordinate and schedule virtual patient appointments, ensuring timely follow-ups
  • Maintain accurate electronic patient records and documentation in compliance with healthcare regulations
  • Collaborate with healthcare professionals to develop and implement individualised care plans
  • Monitor patient progress and communicate updates to relevant stakeholders
  • Assist patients in navigating the healthcare system, including understanding processes related to referrals and available community resources
  • Provide administrative support in a virtual environment, including managing correspondence and maintaining digital records
