Job description

Reablement at Home aims to support people with health and social care needs to stay in their own home rather than being admitted to hospital or into long term care. It also provides support to people when they are discharged from hospital, helps people to remain independent and helps to identify any on-going needs. The service operates 7 days a week, 365 days a year from 7.00am to 10.00pm working in liaison with other healthcare professionals to support those who arein most need, to get them back to living as independently as possible.

Main duties of the job

Job: Rehabilitation Support Worker

Team: Rehabilitation at Home

Salary Range 2: £22,627 under 21; £24,735 21 and over, pro rata per annum plus unsocial hours payments

Flexible working between the hours of 7.00am and 10.00pm, including weekends and bank holidays.

TheRehabilitation Support Worker is a key role in the delivery of our quality serviceand as such we provide an extensive induction and training programme to ensureyou feel empowered to deliver an effective, safe and enabling service to ourservice users.

You will work in partnership with physiotherapists, occupational therapists,nurses and care managers to enable service users to achieve their best qualityof life. Providing short term enablement for Services Users who may need to regain their confidence and skills on discharge from hospital or after a short term health exacerbation at home. We encourage and support our Service Users to live as independent lives as possible and we always act in a manner that supports the care, protection, health and well-being of the Service User. The role requires that you undertake support that has been identified through an assessment process and act upon initial referral information but will respond flexibly to the patient/clients presenting needs. You will also be required to administer medication in accordance with Policy.

Person Specification
Experience
  • Team worker with an appreciation of others skills and expertise
  • Commitment to promoting independence for service users/individuals
  • Is able to assist service users/individuals with social, personal, emotional needs whilst supporting with daily living skills
  • Previous experience of working in a health and/or social care environment
  • Previous personal experience of health and/or social care support
  • Understanding of promoting independence and re-ablement
Other
  • Ability to demonstrate commitment to CPG core values
  • Ability to work weekends and Bank Holiday which will be part of an agreed rota pattern, however this can be changed at short notice to meet the needs of the service
  • Able to work between the hours of 7.00am 10.00pm Monday to Sunday.
  • Ability to transport self, and often equipment, within the boundaries of North East Lincolnshire
Qualifications
