Job summary

East London NHS Foundation Trust is seeking a Rehab Support Worker on a 1-year fixed-term contract to support rehabilitation for patients with neurological conditions, helping them reach independence and quality of life.

You will work within a multidisciplinary CNS team, deliver group and individual therapy activities, monitor progress, keep electronic records, and liaise with therapists, carers and families.

Responsibilities

• Provide encouragement, support and practical help to promote independence and quality of life.
• Work autonomously to carry out rehabilitation plans and review progress with therapists.
• Collaborate with physiotherapists, occupational and speech therapists, and psychologist to support goals.
• Monitor progress during and after treatment and group sessions.
• Lead group exercise programmes and demonstrate exercises to patients and carers.
• Keep clear, concise electronic records and report progress to the MDT.
• Identify and report changes in health condition and ensure follow-up care.
• Assist with referrals, appointments or transport and act as advocate.
