To coordinate care services, conduct utilization review and provide case management for Spain Rehabilitation Center patients in a cost‑effective manner. To orient and support the patient and family to the rehabilitation process. To assess patients’ status. To coordinate and facilitate discharge planning. To effectively transition the patient through the continuum of care.
