Matrix Providers is hiring Registered Nurse Registered Nurse (RN) Patient Care Facilitator (Substance Abuse and Rehabilitation Program (SARP), Behavioral Health (BH), and Traumatic Brain Injury (TBI) Departments)to join our team of talented professionals who provide health care services to our Military Service Members and their families in at Naval Medical Center Portsmouth, VA

  • Employment Status: Full Time
  • Schedule: Services shall be provided Monday through Friday, between the hours of 0630 and 1800 hours. Shifts will be on a rotated basis within the time range specified as clinical demand dictates. Shifts will normally be scheduled for an eight and half (8.5) or nine (9) hour periods, to include an uncompensated thirty (30) minute. In no instance will the HCW be required to provide services in excess of eighty (80) hours per two-week period.

The Registered Nurse must have and meet the following:

  • Degree: Associates Degree of Nursing.
  • Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE), or may be a graduate of a state accredited professional nursing program.
  • Experience: Possess a minimum of two (2) years’ experience in a clinical work setting.
  • Certification: Basic Life Support for Healthcare Providers, American Heart Association, or American Red Cross Healthcare Provider Course.
  • Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse

Registered Nurse Core Job Duties:

  • Assist mental health patients through the TRICARE and MTF health care system, serving as a patient advocate and navigator.
  • Participate in patient-centered Mental Health and Substance Abuse and Rehabilitation Program (SARP) Department team meetings and quality improvement initiatives as assigned. Proactively identifies patients for mental health or substance use disorders case management, care coordination and facilitation in collaboration with the health care team.
  • Provide and/or assist to facilitate patient education with regard to common mental health and substance use disorders and available treatment options at every step in the treatment process from acute hospitalization, residential treatment and in conjunction with all outpatient services closely monitoring and reporting on patient outcomes measures (e.g. HEDIS, ORXY, others).
  • Apply in-depth knowledge of nursing theory and principles relative to the care of patients with complex mental health conditions, and substance use disorders including the use of milieu therapy, group dynamics, psychotropic medications, individual and protocol therapies and crisis intervention techniques.
  • Support the patient with self-management of disease and behavior modification interventions.
  • Coordinate the continuity of patient care with external government health care organizations and TRICARE network facilities and providers to include, but not limited to, referrals between primary care and specialty providers and with appropriate community resources. When required to meet MTF approved patient care needs, the HCW will work with MTF and TRICARE health benefits advisors concerning any non-network referrals.
  • Assist with high-risk patient care, including management of patients with multiple co-morbidities or at high risk for readmission to a hospital setting.
  • Conduct comprehensive preventive screenings for patients and/or assist all support staff in daily patient interactions as needed.
  • Assist in data collection, health outcomes reporting, and programmatic evaluations as part of patient care.
