Job summary

Intermountain Health is seeking a Hospitalist Coordinator RN to support safe, patient-centered care at St. Mary’s Regional Hospital in Grand Junction, Colorado.

You’ll partner with physicians, care managers, nurses, and the care team to coordinate daily care, enhance patient flow, and ensure smooth hospitalization and discharge processes. You will participate in rounds, manage care plans, and help coordinate admissions and discharges while promoting patient engagement and continuous improvement

Qualifications

• Current RN license in the state of practice.
• BSN within three years of hire or promotion if not already completed.
• Two years of clinical hospital experience.
• Basic Life Support (BLS) certification for Healthcare Providers.
• Strong verbal and written communication skills.
• Bachelor of Science in Nursing from an accredited institution. Education will be verified.

Responsibilities

• Coordinate daily patient assignments and census management for the hospitalist team.
• Conduct pre-round preparation and coordinate follow-up needs with care management and ancillary teams.
• Participate in multidisciplinary bedside rounds and facilitate communication among care team.
• Support physician rounding by updating care plans and documenting orders.
• Coordinate discharge planning activities, including follow-ups and equipment needs.
• Act as a liaison between patients, families, physicians, and hospital departments to improve communication.
• Promote patient engagement through education and post-discharge follow-up.
• Support quality, safety, patient experience, and operational improvement initiatives.

Serving as a key partner to physicians, care managers, nurses, and interdisciplinary team members, this position helps coordinate daily patient care activities, facilitate communication, optimize patient flow, and support seamless transitions throughout the hospitalization and discharge process.

This clinically focused role combines nursing expertise, care coordination, patient advocacy, and operational support to help ensure exceptional outcomes and experiences for patients, families, and caregivers.

What You’ll Do

• Coordinate daily patient assignments, patient census management, and workflow support for the hospitalist team.
• Conduct pre-round preparation by reviewing patient information, confirming key demographics, coordinating follow‑up needs, and collaborating with care management and ancillary teams to identify discharge barriers.
• Participate in multidisciplinary bedside rounds, facilitating communication among physicians, nurses, patients, families, interpreters, and other members of the care team.
• Support physician rounding activities by updating patient care plans, coordinating family discussions, documenting verbal orders within scope, and ensuring care team alignment.
• Coordinate discharge planning activities, including follow‑up appointments, durable medical equipment needs, medication authorization support, and transitions of care.
• Serve as a liaison between patients, families, physicians, community providers, pharmacies, and hospital departments to enhance communication and continuity of care.
• Promote patient engagement through education, care plan reinforcement, post‑discharge follow‑up, and resolution of medication or care coordination concerns.
• Support quality, safety, patient experience, and operational improvement initiatives while helping ensure efficient patient flow and exceptional care delivery.

Minimum Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Basic Life Support (BLS) certification for Healthcare Providers.
• Two years of clinical hospital experience.
• Basic computer proficiency.
• Strong verbal and written communication skills.
• RNs hired or promoted into this role must obtain a Bachelor of Science in Nursing (BSN) within three years of hire or promotion if not already completed.
• Bachelor of Science in Nursing (BSN) from an accredited institution. Education will be verified.

Preferred Qualifications

• Experience in hospital‑based care coordination, case management, utilization review, or hospital medicine.
• Knowledge of patient throughput, discharge planning, and interdisciplinary care coordination.
• Experience with quality improvement, process improvement, or clinical operations initiatives.
• Familiarity with electronic medical records and healthcare information systems.

Physical Requirements

• Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.
• Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
• Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.
