A healthcare provider in the UK is seeking a band 5 rotational nurse to deliver holistic care primarily to older adults. The role involves planning and evaluating patient care, supporting junior staff, and maintaining a safe clinical environment in a multidisciplinary team setting. You will work as a band 5 rotational nurse, working between the Community Frailty SDEC and Teign Ward, Newton Abbot Hospital which provides care to predominantly older adults. You will be a vital part of a multi‑disciplinary team. You will provide holistic care from assessment to discharge of your patients, ensuring a safe clinical environment for patients, their carers, visitors and staff and complying with Trust values and expected behaviours.
A new model of care is being introduced for people identified as living with frailty. This model will be based out of a Healthy Ageing Hub at Newton Abbot Hospital and will triage most patients through a dedicated telephone frailty co‑ordination function. Patients will then be streamed to the most appropriate part of the service for their needs, with the option of remaining at home or in a community setting (e.g., intermediate care bed supported by Urgent Community Response teams and/or the Frailty Virtual Ward team). In cases where patients cannot be managed at home or in intermediate care, access to beds at Newton Abbot Hospital will be provided, and the hub team will work proactively to discharge patients as swiftly as possible to care closer to home.
The hub will be a collaboration between existing UCR teams, the Frailty Virtual Ward team, Healthcare of Older People team and the Newton Abbot ward MDT. Staff within these teams include GPs, Advanced Clinical Practitioners (ACPs) and trainee ACPs, therapists, nurses, consultants and a consultant nurse.
