Job summary

A leading healthcare organization in North Wales is seeking a candidate to manage patient assessments and treatments, including risk assessments and care plans. Job description

The successful candidate will be responsible for assessments and treatments patients including risk assessment, care and treatment plans, specific health interventions using evidence-based treatments and health promotion.

The post holder will be based working shifts patients inclusive of unsociable hours. They will use effective communication to keep patients involved in treatment and service contact, and to be responsible for service development and improvement.
