Job summary

Sussex Partnership NHS Foundation Trust is seeking a registered nurse to join the Mental Health Rehabilitation service in Horsham. The role provides holistic, person-centred care to adults with psychosis and complex mental disorders, under supervision of registered nurses.

Mental Health Rehabilitation offers: a whole systems approach to recovery from mental illness that maximises the individuals quality of life and social inclusion by encouraging their skills promoting independence and autonomy in order to give them hope for the future and leads to successful community living through appropriate support. The Sussex Community Rehabilitation Pathway provides high quality, safe and effective rehabilitation for adults with psychosis and complex mental disorder across Sussex. The team works collaboratively with patients who meet NICE guidance service criteria to deliver personalised rehabilitation in their local area to:

• Support a personally meaningful recovery
• Improve subjective quality of life
• Enable social inclusion and participation in the wider community to achieve personal recovery goals

The Sussex Community Rehabilitation Service operates both short and long-term pathways, with short-term pathways temporary community rehabilitation input to people for up to 16 weeks and long-term pathways which offer input to people for an expected minimum of 3 years.

Main duties of the job

• A breadth of knowledge & skills providing holistic & person centred care & support for people of all ages, in a variety of settings
• Works independently under the leadership of registered nurses providing care, delivering all aspects of nursing processes
• Includes undertaking care & treatment tasks as directed by registered nurse within a range of mental health settings with patients experiencing a range of mental health issues. Includes observations & reporting on patients' conditions
• Contribute to collaborative care planning & safety management in accordance with team approach
• Work flexibly & carry out short/long-term intervention case loads
• Carry out delegated tasks in accordance with care plans, risk management plans, with the support & guidance of lead practitioner & wider team
• Keep accurate records of all work undertaken
• Be managerially accountable to Team Leader

Detailed job description and main responsibilities

• Know parameters of medication administration within the speciality they are working in Administer medication within NMC guidelines and the Trust's Medicines Code, within level of competence and job role and ensure they are up to date with their mandatory training
• Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
• Demonstrate good understanding of the Mental Capacity Act (2005)/Mental Health Act (1983/2007)/Deprivation of Liberties and applies principles to everyday practice seeking advice guidance from the Registered Nurse or registered care professional as required.
• Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the on-going assessment, planning, management and evaluation of care
• Support individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence through awareness raising and care navigation to other professionals as appropriate.
• Perform and record clinical observations including blood pressure, temperature, respirations, and pulse.
• Undertake clinical tasks including venepuncture and ECGs
• Accurately record nutritional and fluid intake
• Ensure the privacy, dignity and safety of individuals is maintained at all times
• Demonstrate the ability to recognise changing priorities seeking advice and guidance from the registered nurse or other registered care professionals as appropriate
• Report back and share information with the registered nurses on the condition, behaviour, activity and responses of individuals
• Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals
• Assist in the assessment of and contribute to the management of risk across several areas within the environment where care is being administered
• Assist in the implementation of appropriate action to meet the specific physical emotional and psychological, social, cultural and spiritual needs of individuals and carers
• Assist in the delivery of complex care as prescribed by the registered nurse
• Develop understanding of caring for individuals with particular conditions for example dementia, mental illness, learning disabilities
• Develop care plans based on an individual's needs and that outcome based taking into consideration the needs of the family as appropriate and promote their involvement with the care plan and then ensuring that an registered nurse has signed it off.
• Develop skills in relation to coaching/teaching individuals/carers/other staff
• Assist with the implementation and monitoring of clinical standards and outcomes. Develop a working knowledge of other providers' resources and referral systems to ensure individual's needs are met, within parameters of practice
• Engage in reflective practice including management of self and reflection on own reactions, asking questions and reflecting on answers given
• To promote respect for diversity and combat stigma and exclusion
