Job Summary

Key Responsibilities

As a Band 5 Nurse you will assess and develop plans of care that enable patients to achieve their optimum level of self‑dependency, recognise individual needs, and support rehabilitation.

  • Deliver and agree on patient plans of care using a holistic approach.
  • Evaluate plans of care and liaise with the multidisciplinary team as appropriate.
  • Plan discharge of patients according to individual assessment.
  • Refer and liaise with statutory and voluntary agencies, acting as an advocate for patients.
  • Participate in direct patient care and improve the quality of the patient journey and experience.
  • Administer drug therapy ensuring the safe custody of medication.
Qualifications
  • Registered Nurse, NMC qualification.
  • UK professional registration current.
Experience & Skills
  • Demonstrated working knowledge of evidence‑based practice.
  • Ability to assess and evaluate complex health conditions.
  • Experience in formulating assessments, planning, coordinating, and supporting patients in a hospital setting.
  • Experience providing care and interventions within a health setting.
  • Aware of multidisciplinary working and ability to work across professional boundaries.
Additional Information

Applicants must have current UK professional registration. A Disclosure and Barring Service check will be required, and applicants should be prepared for a DBS submission.

This role is a 24/7, 365 shift‑based position, with flexible working considered where service needs allow. You may work across both Haywood and Cheadle Hospitals.

We offer an induction package and preceptorship programme for newly qualified nurses, and a full‑time or part‑time band 5 working pattern.
