South Hams Hospital Ward is a supportive environment for developing your skills in a rehabilitation setting. We invite you to visit the hospital for an informal visit. South Hams Hospital is located approximately 25 miles from Plymouth in Kingsbridge, near the estuary and close to lovely beaches. All communication will be electronic, so check your email regularly.

Role Overview

You will work under the direction of the Nurse‑in‑Charge and as part of a multidisciplinary team to coordinate and deliver effective quality nursing care for patients in rehabilitation or end‑of‑life settings.

Key Responsibilities
  • Provide and deliver a high standard of nursing care by initiating, co‑ordinating, and evaluating individual patient needs based on evidence.
  • Report and document all relevant nursing/patient needs, communicating changes with other health care professionals and maintaining responsibility for patient care as it occurs.
  • Engage and interact with carers and family, encouraging active involvement in patient care planning as outlined in ward/unit philosophy.
  • Promote and deliver health education within the clinical area and advise patients’ families and/or significant others on health promotion issues.
  • Demonstrate and undertake evidence‑based practice, playing an active part in the introduction of evidence to improve patient‑centred care.
  • Supervise and direct Health Care Assistants with their daily workload.
  • Communicate with patients, relatives and outside agencies as appropriate, within the confines of the Data Protection Act and Patient Confidentiality.
  • Implement and share good infection control practice.
  • Undertake manual handling assessments and ensure identified actions are met.
Qualifications & Preferences

Preference will be given to internal Trust staff and to individuals with "Priority" or "At Risk" status, including NHS At Risk staff throughout Devon, who can demonstrate recent relevant experience as required by the Person Specification.
