Job description

A great opportunity has risen for a Registered Nurse to join our ward team at Tavistock Hospital, University Hospitals Plymouth. The post holder will be responsible for implementing, co‑ordinating and delivering effective, high quality care for the pre‑operative patient, following a client‑centred model of practice and ensuring the area of work runs smoothly and efficiently. To make appropriate verbal and written assessment to gain information relating to a patient's surgery and act on such assessments in preparing and supporting the patient's pathway, referring to outside specialist help if required. Previous demonstrable experience within health care of the elderly care is an advantage but not essential as full training will be given. The successful candidate will work as part of the nursing team to provide high standards of care to ensure a positive experience to all patients.

Responsibilities
  • Provide and deliver a high standard of nursing care by initiating and co‑ordinating the assessment, planning, implementing and evaluating of individual patient needs based on evidence. This includes all aspects of care from admission to discharge.
  • Report and document all relevant nursing/patient needs, communicating changes with other health care professionals in the multi‑disciplinary team with ongoing responsibility for patient care as they occur.
  • Engage and interact with carers and family, encouraging them to be actively involved in patients' care planning as outlined in ward/unit philosophy.
  • Demonstrate and undertake evidence‑based practice, playing an active part in the introduction of this evidence to improve patient‑centred care.
  • Assist other clinical areas if the clinical situation or staffing levels demand.
  • Supervise and direct Health Care Assistants with their daily workload.
  • Communicate with patients, relatives and outside agencies as appropriate, within the confines of the Data Protection Act and Patient Confidentiality.
  • Implement and share good infection control practice.
  • Manage the clinical risk relating to individuals or a group of patients' care, reporting issues to appropriate senior staff.
  • In some circumstances take charge of the ward in the absence of more senior staff.
