The Sidmouth Hospice at Home Community Nursing Team deliver 24hour specialist end of life care to patients and their families during their last year of life in the Sid Valley. The successful candidate will be welcomed into our caring team, working alongside our sister charity, Sidmouth Hospice at Home, and will be based at the charitys new building in Sidford.

As a specialist palliative care nurse, you will have on-going access and be positively encouraged to engage in your own learning and professional development, at your own pace and with supportive clinical supervision.

You will be part of the Specialist Palliative Care Team at the Royal Devon.

More detail about the role

The Sidmouth Hospice at Home service is funded by the charity, and provided by the charity in partnership with the Royal Devon. The Sid Valley Integrated Model for Specialist Palliative Care.

Integrates the available services within the community into a one-team approach to provide the best possible care for all patients who are diagnosed by their GP as palliative, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their carer.

The service is part of community care provision in Sidmouth for patients registered with a Sidmouth GP. You will need to have worked within the palliative care field. Community experience is desirable but not essential. You will have opportunity to learn and gain experience through clinical reflective practice and continued professional development.

You will be joining a professional, specialist palliative care team, who work alongside our health and social care colleagues both in the community and the acute care sectors.

Working Pattern: The service provides specialist palliative care 7 days a week, 365 days a year, with shifts covering 08.00-16.00, 10.00-18.00 and 12.00-20.00. After 20.00hrs, as a Clinical Nurse Specialist you will be part of our on-call service to respond to our patients who need our advice or a visit overnight, thus providing specialist end of life care and carer support 24/7, and avoiding unnecessary and unwanted hospital admissions. Since its beginning in January 2021, the service has enabled 95% of our patients to be cared for by their families at home as their preferred place of death.
