Key Responsibilities
  • Conduct thorough assessments of patients' health status and care needs.
  • Administer medications and treatments as prescribed, ensuring adherence to care protocols.
  • Develop, implement, and evaluate individualized care plans in collaboration with healthcare teams.
  • Provide education and support to patients and families regarding health issues and care management.
  • Monitor and document patient progress, reporting any changes in health status to appropriate personnel.
  • Ensure compliance with all regulatory and safety standards in patient care.
  • Participate in training and mentorship for junior staff and care assistants.
Requirements
  • Current and valid Nursing registration with the NMC (Nursing and Midwifery Council).
  • Proven experience as a Registered Nurse, preferably in a community or homecare setting.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative healthcare team.
  • Current Basic Life Support (BLS) and other relevant certifications.
  • Enhanced DBS check (or the willingness to obtain one).
  • A valid driver's license and access to a vehicle may be required for community visits.
