Job summary

Case Management Cymru Ltd. in Wales is seeking an enthusiastic, reliable support worker to assist a young adult with complex health needs in daily living tasks.

You will help with personal care, mobility, and engagement in activities such as bowling, cinema and music concerts, while also supporting therapy appointments and rehabilitation plans. You will join an expert multi-disciplinary team led by the case manager and parents, with induction and ongoing supervision.

Qualifications

• Experience of working with personal care needs.
• Experience with people with significant communication impairments.
• Be able to drive and have use of a car; willing to drive Motability van.
• Ability to work hours flexibly.
• Willing to register with Social Care Wales.
• Comfortable working in a household with cats.

Responsibilities

• Assist with activities the client loves, such as bowling, cinema and music concerts.
• Support attendance at therapy appointments and rehabilitation programme.
• Remain calm and patient under pressure, adapting to changing needs.
• Be part of a multidisciplinary team led by the case manager and parents.

Job description

I am23years old and have complex health needsdue to a diagnosis of cerebral palsy.I need 24/72:1 support in all areas of daily life and require lots of supportdailywith personal care, communication and mobility.I really enjoy being around other people and I have a great sense of humour. Those who know me best, describe me as ‘a bit cheeky, knowing my own mind, great fun and loving’.

I am lookingfor enthusiastic, fun,patientand reliablesupportworker to help me engage in the activities Ilove such asbowling, the cinema and music concerts.I also need support to attend therapy appointments and complete my rehabilitation programme. You will need to be calm and patient under pressure withthe ability to work your hours flexibly. Youwill be part of my amazing team which is led by my parents and case manager.A full induction and training will be provided. This role is subject to an Enhanced level DBS.

ESSENTIAL INFORMATION To join me as a Support Worker it is essential you:

• Have experience of working with young adults who have personal care needs.
• Have experience of working people with significant communication impairments.
• Be able to drive and have use of a car and be willing to drive my Motability van.
• Have the ability to work your hours flexibly.
