About The Role

We're looking for a dedicated Substance Misuse Practitioner to join our integrated healthcare team, supporting prisoners aged 18+ with drug, alcohol, and low-level mental health needs.

You’ll deliver tailored, trauma-informed interventions, from 1:1 sessions to group programmes, helping individuals prepare for release and reintegrate into the community with confidence and support.

What You’ll Do
  • Deliver structured 1:1 and group sessions for substance misuse and mental health
  • Create and manage care plans, assessments, and harm reduction strategies
  • Work closely with housing, employment, healthcare, probation, and more
  • Support smooth transitions from custody to community
  • Keep accurate records to track outcomes and meet KPIs
