Qualifications

• Previous experience as a Band 2/3 HCA or equivalent in a care environment.
• Previous experience delivering physical health interventions such as taking blood pressure.
• Previous experience working with patients with mental health difficulties.

• Deliver high quality patient focused care in a community setting.
• Promote patient independence and provide physical health interventions.
