Job summary

Dorset HealthCare University NHS Foundation Trust invites applications for a Community Mental Health Practitioner in Wareham. You will manage a dedicated caseload of patients with Serious Mental Illness, delivering person-centred, evidence-based interventions to support recovery.

You’ll work within an MDT with Consultants, nurses, therapists, and social care professionals, receiving supervision and opportunities to develop skills in therapies and advanced nursing practice.

Qualifications

• Experience delivering person-centred, evidence-based interventions.
• Working knowledge of biopsychosocial models of care.
• Willingness to work within an MDT with supervision and development.

Responsibilities

• Deliver clinical care and targeted interventions to aid recovery and wellbeing.
• If Registered Nurse, rostered to Depot Clinic and assist with phlebotomy where trained.
• Mentor and assist in induction and training of new staff.
• Maintain links with Inpatient and Crisis Services and ensure clear communication with families.
