Job summary

Hartlepool Council is seeking a dedicated care coordinator to support the leadership team in developing and delivering a substance misuse service. You will be responsible for the assessment, planning, implementation and evaluation of care for a caseload of individuals, encouraging access to services and promoting recovery for individuals, families and communities.

Fixed term contract to 31 March 2027, 37 hours per week, with potential for further funding, and opportunities for supervision,

Qualifications

• Experience of working with vulnerable individuals with multiple needs is required.
• Caseload management and coordinating care across multiple services.
• Delivery of evidence-based individual and group psychosocial interventions.

Responsibilities

• Assess, plan, implement and evaluate care for a caseload of service users.
• Support service users to access services and develop their strengths for recovery.
• Coordinate and deliver psychosocial interventions and recovery-focused support.
