Conduct comprehensive assessments of service users’ needs, risks, and strengths.
Develop personalised recovery plans with clear goals and evidence-based interventions.
Deliver harm reduction advice, motivational support, and relapse prevention strategies.
Manage a varied caseload, supporting individuals with different levels of complexity.
Facilitate group sessions, psychoeducation, and peer-led activities.
Work closely with mental health teams, housing providers, and external partners.
Identify, monitor, and address risks relating to substance misuse, safeguarding, and wellbeing.
Maintain accurate records in line with confidentiality, data protection, and organisational standards.
Experience managing a caseload within substance misuse or a related setting.
Previous work with individuals experiencing addiction, homelessness, or mental health challenges.
Strong understanding of harm reduction, recovery models, and trauma-informed practice.
NVQ Level 3 in Health & Social Care (or equivalent) is highly desirable.
