  • Be a visible, accessible and professional authoritative presence for patients and staff, in service base, MDT and county-wide clinics.
  • Work daily in direct clinical contact with patients and their families to ensure high standards of physical health care and timely medical response when required.
  • Provide holistic nursing assessment and high quality, safe, evidence-based care to patients.
  • Assess, diagnose, care plan, and review all aspects of physical care.
  • Contribute to continuous improvement by identifying day-to-day changes to enhance patient care within community teams.

The Adult Eating Disorder Team has a average caseload of 210 clients and the bespoke physical health pathway is commissioned to provide physical health monitoring and response to the top most high risk clients within it. The physical health caseload is fluid and therefore will give the successful candidate the opportunity to make a difference to many of our clients recovery journey's throughout their work with us.

You will be in good company too, as your immediate colleagues include other physical health nurses, dietitians, a nurse Lead, Consultant GP and Specialist Registrar. With us you will have the opportunity to be running clinics across the county with autonomy, whilst maintaining strong links with your immediate colleagues, as well as having overall support from and being embedded with the wider Multi-disciplinary team.

If you bring the physical health nursing skills and a passion to learn and develop, we will support you to build your knowledge and expertise in the rewarding and dynamic field of mental health care.

  • Be a visible, accessible and professional authoritative presence for patients and staff, in both the service base, MDT and running clinics automonsouly across the county.
  • Working daily in direct clinical contact with patients and their families within the community to ensure high standards of physical health care are being carried out and physical health safety of our client's is being effectively monitoring and responded to when medical response and/or rescue is required.
  • To ensure provision of holistic nursing assessment and high quality, safe, evidence based care to patients.
  • Assessing, diagnosing, care planning, reviewing all aspects of physical care
  • The post holders will also be encouraged and supported to make improvements to care that they identify on a day to day basis.
  • Help carry out audits to ensure that practise is safe and analyse data and formulate strategies to improve practice within the community teams with the staff.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
